Terms of service
Version: 2026-10-08
These Terms govern use of formreminder.com (the Site) and the FormReminder application (the App), together the Services, provided by 1423 Interests LLC (we, us).
Part A applies to everyone who uses the Site. Part B also applies if you create an account or use the App.
If you create an account, you accept these Terms by checking the agreement box at sign-up or checkout. You accept them for yourself or, if you are signing up for an organization, on behalf of that organization (you). You must be at least 18 years old to create an account.
People who receive reminder emails from our customers are not parties to these Terms. Their information is covered by our Privacy Policy and by the unsubscribe link in each email.
Part A: Everyone who uses the Site
1. The Services
FormReminder helps organizations follow up on online forms. It tracks who has submitted and sends automatic email reminders only to those who have not. We may add, change, or remove features over time. If a change materially reduces what the App does for paying customers, we will give reasonable notice.
We may discontinue the Services. If we do, we will give account holders at least 30 days' notice and refund the unused portion of any prepaid fees.
2. Ownership and license
The Services, including the software, design, text, and logos, belong to us or our licensors. We grant you a limited, non-exclusive, non-transferable right to use the Services as these Terms allow. Do not copy, resell, or reverse engineer the Services except as the law permits. If you share feedback or suggestions, we may use them to improve the Services without obligation to you.
3. Third-party services
The Services work with Google Forms and may work with other third-party form builders and tools. Those services are governed by their own terms. We are not responsible for them, or for changes to them that affect how FormReminder works.
4. Disclaimers and limits of liability
The Services are provided as-is and as-available. To the fullest extent the law allows, we disclaim all warranties, express or implied, including implied warranties of merchantability, fitness for a particular purpose, and non-infringement. We do not guarantee uninterrupted or error-free operation, or that any email will be delivered, opened, or acted on.
The Services help you follow up on forms. You remain responsible for meeting your own deadlines and your legal and regulatory obligations.
To the fullest extent the law allows, we are not liable for indirect, incidental, special, consequential, or punitive damages, or for lost profits, revenue, or data. Our total liability for all claims relating to the Services is limited to the greater of (a) the fees you paid us in the 12 months before the claim or (b) $100.
Some laws do not allow certain disclaimers or limits. Where that is the case, this section applies only as far as the law permits.
5. Changes to these Terms
We may update these Terms and will post the new version with its date. Material changes will be emailed to account holders at least 30 days before they take effect. If you keep using the Services after an update takes effect, the updated Terms apply to you. If you do not agree with an update, stop using the Services and cancel your account.
6. Governing law and contact
These Terms are governed by the laws of the State of Texas, without regard to its conflict-of-law rules. Any dispute relating to these Terms or the Services will be brought only in the state or federal courts located in Dallas, Texas, and you and we consent to those courts.

7. General terms
- Entire agreement. These Terms, our Privacy Policy, and any plan details shown at checkout are the entire agreement between you and us about the Services.
- Severability. If a court finds part of these Terms unenforceable, the rest stays in effect.
- No waiver. If we do not enforce a provision, we have not given up the right to enforce it later.
- Assignment. You may not transfer these Terms without our written consent. We may transfer them to an affiliate, or to a successor in a merger, reorganization, or sale of the FormReminder business, and we will notify account holders if we do.
- Events beyond our control. We are not responsible for delays or failures caused by events outside our reasonable control, such as outages at hosting, email, or form providers.
Part B: Account and App use
8. Your account
You decide who in your organization has access to your account. You are responsible for activity under your account and for keeping login credentials secure. Tell us promptly if you suspect unauthorized access. If you create an account for an organization, you confirm you have authority to accept these Terms for it.
9. Connecting your Google account
To use the App, you connect your Google account and choose the Google Forms for FormReminder to monitor. You authorize us to access only the forms you choose and their responses, and only to determine who has submitted. You may disconnect at any time, including from your Google account settings. How we handle information received from Google is described in our Privacy Policy.
10. Your Data and form responses
You own the data you or your users provide, including invitee lists and account information (Your Data).
Your form responses stay in your Google Form. We read responses only to match submissions to your invitee list. We do not copy or keep the answers people give on your forms. We keep only what is needed to track status.
We use Your Data only to provide, support, and improve the Services for you. We do not sell it or share it with third parties, except service providers that host or operate the Services (such as email delivery providers) under confidentiality obligations, or where required by law.
Keep invitee information to what is needed, usually names and email addresses. Do not enter sensitive personal information, such as Social Security numbers or financial account numbers, into invitee lists or other fields.
11. Reminder emails
You choose the form and the invitees. We send reminders on your behalf to invitees who have not yet submitted. You are the sender of record for these reminders in your relationship with the invitees. By adding an invitee, you confirm that:
- You have a legitimate existing relationship with that person, such as a member, student family, employee, volunteer, or donor, and the right to email them about the form.
- The email address was not purchased, rented, or scraped.
- You have any consent or authority required by law, including where an invitee is a minor.
Each reminder includes an unsubscribe option. We will stop sending to anyone who unsubscribes, and we may stop sending to anyone who reports a message as spam. An unsubscribe applies to [all reminders from your account]. Do not ask us to email them again.
Reminders must relate to the form you asked the invitee to complete. Email can be delayed, filtered, or blocked, and we cannot guarantee delivery or response. You remain responsible for following up when a deadline matters.
12. Prohibited use
Do not use the Services to:
- Break the law, including email, privacy, and consumer protection laws that apply to you or your invitees.
- Send spam, advertising, or content unrelated to the form you asked invitees to complete.
- Email people from purchased, rented, or scraped lists, or people you have no legitimate relationship with.
- Impersonate another person or organization, or send misleading, harmful, or phishing content.
- Access other customers' data, disrupt the Services, or get around usage limits.
We may limit or suspend sending, with or without prior notice, if we see spam complaints, high bounce rates, or other abuse that puts recipients, email deliverability, or the Services at risk.
13. Security
We use reasonable administrative and technical measures to protect Your Data. No system is perfectly secure, and we will notify you promptly if we learn of unauthorized access to Your Data.
14. Data deletion and retention
You may delete your invitee lists and form settings at any time. When you stop using the Services, we will delete Your Data within 30 days of your request, except where retention is required by law or backup cycles. We keep a record of email addresses that have unsubscribed or reported spam, so we can keep honoring those opt-outs. Responses submitted to your forms remain in your Google Form.
15. Fees and cancellation
Fees are shown at checkout or in your order and do not include any taxes that may apply. Subscriptions renew automatically each billing period, monthly or yearly, until canceled, and we charge your payment method on file at each renewal. For yearly plans, we will email a reminder before renewal.
You may cancel anytime [in your account settings or by emailing us]. Cancellation takes effect at the end of the current billing period, and fees already paid are not refunded, except as stated in Sections 1 and 18. We will give at least 30 days' notice before changing your price.
16. Free trials and early access
We may offer a free trial or early access. During that period:
- Features may change, and occasional errors or downtime are more likely.
- Paid terms begin only when you choose to subscribe.
- We may change or end trial and early access offers.
17. Indemnity
You will defend and indemnify 1423 Interests LLC and its owners, employees, and contractors against third-party claims, and related costs including reasonable attorneys' fees, that arise from:
- Your Data, including your invitee lists.
- Emails sent on your behalf through the Services.
- Your violation of Section 11 or 12, or of any law.
We will notify you promptly of any such claim and cooperate reasonably in its defense.
18. Termination
Either party may end this relationship at any time. We may suspend access for non-payment or misuse after reasonable notice, or immediately where Section 12 applies. If we end your account for a reason other than non-payment or misuse, we will refund the unused portion of any prepaid fees.
Sections 2, 4, 6, 7, 10, 14, and 17, your confirmations in Section 11, and any fees you owe survive termination.
